To request a sick note:
- phone or visit the surgery from 10am to 6pm
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Do I need a return to work note?
If you are fit for work, you do not need a fit note.
Your doctor will only give you a fit note if your health affects your fitness for work.
Can I get a fit note without seeing my GP?
A fit note must be signed by a doctor, but you do not always need to see a GP in person to get one. Please see NHS website for details.
